Is action required?

YES!! Enrollment is required. It is important that everyone take the time to review and understand their options and make choices this year.

Must I enroll online?

Yes, this year online enrollment is required.  Please see your manager or site administrator if you need assistance with accessing a computer.

Can I switch to my spouse's plan during the year?

If you experience a qualifying life event, such as a change (gain or loss) in your spouse's eligibility for coverage through their employer, you may make changes if you notify us within 31 days.  If your spouse goes through their annual Open Enrollment process, you may join their plan and drop Inframark coverage at that time.


When will we get new Aetna Medical ID cards?

New Aetna Member ID cards will be uploaded electronically to all member profiles effective January 1st of every new year. You may also register for Aetna Navigator on or after January 1st.

To print/order/access your Aetna Medical ID card, visit the Aetna website and login.

How do I find a participating healthcare provider?

Visit Aetna.com/docfind, select the provider, enter your zip code, select "Aetna Premier Care Network""2021 Plan Providers", and then "2021 Aetna Premier Care Network (APCN) - Choice POS II" as the plan.  You can also contact Aetna Member Services at 1-888-215-8485 for assistance.

Why would I use the Teladoc service?

The Teladoc benefit gives you access to board-certified physicians to get fast treatment (including prescriptions) for common ailments such as the flu, allergies, ear infections, and more. If you are enrolled in any of the PPO medical options, you will pay nothing for virtual visits.  HDHP enrollees will be charged $47 for virtual visits, which is still considerably less than an emergency room or urgent care visit. 

Teladoc also offers mental health services that allow you to speak with licensed counselors, therapists, psychologists, or psychiatrists by  phone, web, or mobile app.

​Can I use Teladoc for a work-related injury?

NO.  You should follow Inframark policies for reporting and treating a work-related injury.  Please contact your manager for more information.


How can I find a participating dentist?

Visit www.unitedconcordia.com. Click on "Find a Dentist".  Select the provider type and enter your zip code.  Select the "Advantage Plus 2.0" network.

Can I still use my non-participating dentist?

Yes.  However you will always maximize your benefits and minimize your costs by choosing a participating provider.  Also note that if you choose the Low Plan, out-of-network reimbursement rates are further reduced.  Finally, if you use a non-participating provider, you will be responsible for payment and United Concordia will reimburse you, not the dentist directly.

Can I choose the High Plan and enroll my spouse in the Low Plan?

No, it is not possible to enroll different members of the same family unit in different plans.


Who can participate?

Benefits-eligible employees and their families may use Health Advocate services, including wellness, regardless of whether they are enrolled in our Inframark health plans.  However, only employees and spouses who are enrolled in the Inframark health plan will receive incentives for participating.


How much can I contribute to a Health Savings Account (HSA)?

The maximum amount is set each year by the IRS.  For 2021, you can contribute $3,600 if you have single coverage, or $7,200 if you cover dependents.  The contribution that Inframark makes to your account ($500/$1,000) is included in this maximum.
